Welcome to sach community hub

A culturally-sensitive response aimed at meeting the needs of our diverse and vibrant community.

The SACH Process

A key component of this unique model is the centralized entry point, where each individual has their own Case Manager that will facilitate the services based on the needs and advocate on behalf of the individual and family. The service providers and services will be available on site, easily accessible, and all service providers will work together collaboratively to achieve successful outcomes.

    WHO WE ARE

We work together to help you

SACH is a multi-sectoral collective that came together in early 2020 to address key challenges impacting members of our community that are of diverse backgrounds and unmet needs. Together we employ our varied backgrounds, academic and professional experiences to address these needs through cross-sectoral collaboration.
