The SACH Process

A key component of this unique model is the centralized entry point, where each individual has their own case manager that will facilitate the services based on the needs and advocate on behalf of the individual and family. The service providers and services will be available on site, easily accessible, and all service providers will work together collaboratively to achieve successful outcomes.

Matched with a Case Manager who will perform a needs assessment and connect clients directly to services at SACH or through community agencies

Services related to: substance use treatment and harm reduction, counselling, medical services, income assistance, employment, food security, housing, personal hygiene, immigration, recreation, and victim services as well as opportunities for social connection and volunteering.

SACH is a multi-sectoral collective that came together in early 2020 to address key challenges impacting members of our community that are f diverse backgrounds and were having their needs go unmet. Together this collective employs their varied backgrounds, academic and professional experiences to address these needs through cross-sectoral collaboration.

    Our Acknowledgement
